CarePatrol Baltimore Partners with PocketRN to Expand Dementia Support Through the GUIDE Program

At CarePatrol Baltimore, our mission has always been to help families navigate senior care decisions with clarity, compassion, and confidence. We are proud to introduce a new partnership that strengthens that commitment even further.
We are now partnering with PocketRN to support eligible families through the GUIDE Model — a Medicare Innovation Center model designed to improve care for individuals living with dementia while providing meaningful support to their caregivers.
What Is the GUIDE Program?
The Guiding an Improved Dementia Experience (GUIDE) Model is a nationwide Medicare initiative created to enhance dementia care through comprehensive coordination and caregiver support.
The program is designed to:
- Improve quality of life for individuals living with dementia
- Reduce caregiver stress and burnout
- Strengthen communication between healthcare providers
- Provide access to respite services when eligible
- Support individuals in remaining safely at home longer
Nearly 7 million Americans are currently living with Alzheimer’s disease, and that number continues to rise (Alzheimer’s Association). Dementia care can often feel fragmented and overwhelming for families. The GUIDE Model helps address this by offering structured, proactive support rather than crisis-based intervention.
How This Partnership Helps Families in Baltimore
Through our partnership with PocketRN, families are supported in every step of the GUIDE enrollment and care process. CarePatrol Baltimore is the local contact for home assessment. The PocketRN process is as follows
✔ Determine Eligibility
We help identify whether your loved one may qualify for this Medicare-approved program by securely coordinating eligibility verification. You will need to be covered by Traditional Medicare Parts A and B (Medicare Advantage plans are not eligible), and an unpaid caregiver is typically involved, particularly for caregiver support services. You cannot be in a long-term care facility nor receive hospice services.
✔ Navigate Important Documentation
Families receive support gathering essential documents such as:
- Medical records confirming a dementia diagnosis
- Medication lists
If documentation is not yet available, additional evaluation and cognitive screening by PocketRN may be arranged to help move the process forward.
In-home assessment: CarePatrol Baltimore will complete a home assessment and assessment of activities of daily living. CarePatrol supports home safety and functional assessments, while PocketRN conducts the comprehensive clinical assessment.
✔ Receive a Comprehensive Nursing Assessment
Eligible clients complete a thorough assessment with a nurse experienced in dementia care. This evaluation reviews the in-home assessment, health status, medications, safety concerns, and caregiver needs to build an individualized care plan.
✔ Ongoing Care Coordination
Once enrolled, participants benefit from structured nursing oversight, medication review, and consistent follow-up support. You are assigned a dedicated nurse through PocketRN’s Nurse For Life model.
✔ Access Respite Services (If Eligible)
Caregiver strain is one of the most significant challenges families face when caring for a loved one with dementia (Alzheimer’s Association). For families who qualify, the GUIDE Model includes respite support — providing caregivers with scheduled relief and helping prevent burnout.
What Makes GUIDE Different?
Unlike traditional healthcare models that often operate in silos, the GUIDE Model centers on coordinated dementia-specific care. It focuses on multiple domains of health and well-being — not just medical treatment, but safety planning, caregiver education, medication management, and long-term stability.

The process typically includes:

- A welcome call and medication review

For families who already have a documented dementia diagnosis, enrollment times may vary. If documentation is still being gathered, timelines may be longer. Services are provided through PocketRN under the GUIDE model.
